About the Role
The Older Persons’ Mental Health Community Team provides psychiatric and geriatric care for the older members of our community. We aspire to provide high quality care to patients in the community with mental health needs and/or complex age-related health conditions. We work in partnership with our patients, their whanau/families and community supports to achieve the best possible health outcome for the patient.
About the Team/Service/ Location
Based in Richmond, the team provides short term case management for people of any age with dementia who have significant behavioural symptomology, and people with a mental disorder aged 75 or older. We aim to foster autonomy, independence and quality of life while recognizing the importance of whanau to the client’s wellbeing. The case managers role involves overseeing the response and tolerance to medications while providing non-pharmacological support and linking in with wider community supports. The community team is the gateway to the inpatient service which allows for a range of community-based interventions to be explored prior to inpatient admission. We work closely alongside the inpatient team to ensure best practice, holistic care, and to provide support on discharge. Our team includes case managers, Clinical Nurse Specialist, an ARC liaison nurse, Occupational Therapist, Social worker, Psychologist, Old Age Psychiatrist and physiotherapist. We operate a Monday-Friday 0800-1630 service.

About you
You will have current NCNZ registration and practising certificate, with no conditions that exclude Mental Health Scope of Practice, and a minimum of two years post-registration nursing experience is desirable.
You will have knowledge of:
- Physical and mental disorders commonly experienced in older persons
- Contemporary treatment approaches including person-centred care, sensory modulation, therapeutic use of self and therapeutic use of psychiatric medications
- The role of community-based case management
- Mental Health assessment and care planning
- Trauma informed care
- Mental Health (Compulsory Assessment and Treatment) Act, 1992
- PPPR Act - Welfare Guardianship
Your skills will include:
- Well-developed interpersonal skills
- Demonstrated ability to work in partnership with vulnerable populations
- Ability to work effectively in multi-disciplinary team and to work autonomously
- Ability to work effectively with patients who have complex and diverse needs.
