About the Role

  • Provide specialist clinical assessment and intervention for older and frail patients in the Emergency Department.
  • Act as a key resource for colleagues, offering advice and support in the management of complex needs.
  • Work closely with multidisciplinary teams including geriatricians, therapists, social care, and community partners to deliver safe and effective care pathways.
  • Contribute to service development, innovation, and quality improvement projects focused on frailty care.
  • Support education and training for nursing and medical staff on the recognition and management of frailty.

About you

We are now seeking a Clinical Nurse Specialist (CNS) with expertise, or a strong interest, in elderly and frailty care to join our Emergency Department team. This is an exciting opportunity to make a real impact on the patient journey, reducing unnecessary admissions, improving outcomes, and supporting patients and families at a time of real need.

  • Registered Nurse with significant clinical experience, ideally within emergency, acute, or frailty settings.
  • Strong communication and leadership skills, with the ability to influence and inspire others.
  • Passion for improving the care of older people and those living with frailty.
  • Evidence of ongoing professional development, with or working towards specialist/advanced qualifications.
  • Commitment to collaborative working across teams and services.
