Clinical Needs Assessor – Health of Older People / Chronic Conditions

Ngā mōhiohio e pā ana ki tēnei mahi (About this position)
Working within a Restorative Care model, our Needs Assessment team works with older people with a health related disability, or those under the age of 65 affected by complex health conditions. Together we identify goals to assist them towards safe, independent living so they can enjoy life the way they want to.
Our team based at the Richmond Hub and we have close working relationships with Nelson Hospital, other health agencies and community services.

We are looking for a motivated and enthusiastic registered health professional to join our team. Ideally you will be a Registered Nurse, Physiotherapist or Occupational Therapist, however other health professionals will be considered.
We will value your commitment and experience in working with people and their families to ensure that their needs are at the centre of their care.
