Clinical Coordinator – Early Support Discharge

About the Role
As the Clinical Co-ordinator in the Early Support Discharge Team, you will play a key role in leading the team while also working alongside them in your clinical role, to help patients regain mobility and independence after hospital discharge. You will work within a multidisciplinary team to deliver person-centred care in home and community settings.
You will work in a collaborative setting, sharing expertise and resources to deliver the best possible outcomes for our patients. You will play a key role in assessing, planning, and delivering individualised treatment programs. You will support a variety of outpatient cases, collaborating closely with multidisciplinary teams to provide holistic and effective support.
The working hours are between 7am-7pm Monday to Sunday.

About the Team/Service
An Early Support Discharge Service is being established in the Nelson Tasman region, designed to help patients transition smoothly from hospital to home with the right support.
The team will work closely with patients, whānau, and healthcare providers to improve recovery outcomes and independence in the community. We are seeking passionate professionals to join our dynamic team and make a real difference in people's lives.
About you
- Competency with te ao Māori, tikanga, and te reo Māori or a commitment to starting your journey and taking ownership of your learning and growth
- Experience in projects / initiatives which give effect to Te Tiriti principals and frameworks, and the application of Mātauranga Māori and Kaupapa Māori approaches, particularly as they apply in healthcare settings
- A registered health professional with a minimum of 5 years clinical experience
- Experience of working with other professions and MDT processes
- Contract monitoring experience
- Excellent interpersonal skills, a can do attitude with a strong quality focus
- A capable leader but also a strong team member who is enthusiastic and innovative
