Needs Assessor - Health of Older People, Marlborough

About the role

As a Needs Assessor, you will play a critical role in evaluating and coordinating care plans for older persons experiencing health challenges or requiring support to maintain their independence. You will work collaboratively with individuals, families, and multidisciplinary teams to ensure that clients receive tailored support aligned with their health and wellbeing goals. You’ll have the opportunity to make a meaningful impact in the lives of older adults while working in a supportive and collaborative environment.

  • Permanent 0.5FTE role: focused on long-term service delivery for older persons in the community
  • Fixed term 0.3FTE role: covering leave, this position supports continuity of care and seamless service delivery.

About the Team/Service/Location

Marlborough Health Hub provides a vital service dedicated to improving the health and wellbeing of older adults in our community. We are committed to delivering compassionate, person-centred care that empowers older persons to live independently, maintain their dignity, and enhance their quality of life.

If you are enthusiastic and experienced Needs Assessors who are either registered Occupational Therapists, Social Workers, Psychologists, or Registered Nurses with experience working with older adults, we would love to hear from you. You will have close working relationships with the Nelson Needs Assessment team, Nelson hospital, and will work in partnership with Te Piki Oranga Māori Health Providers, community services, inter-district health services and Iwi.

About you:

The successful candidate will:

  • Conduct comprehensive assessments of clients' health, social, and functional needs in line with government guidelines.
  • Develop, implement, and review personalised care plans in collaboration with clients, families, and healthcare providers.
  • Work closely with multidisciplinary teams to ensure appropriate services and support are provided.
  • Advocate for clients, ensuring their needs are prioritised and their voices heard.
  • Maintain accurate documentation and reports in compliance with relevant policies and standards.
  • Provide education and guidance to clients and families about available resources and support systems.

Skills and experience

  • Registered Nurse or Allied Health Professional.
  • Proven experience in needs assessment and service coordination, preferably in older persons' mental health or disability support services.
  • Strong understanding of the needs of older persons and knowledge of community health resources.
  • Excellent communication and interpersonal skills, with the ability to build rapport and trust with clients and families.
  • Ability to work effectively in a multidisciplinary team and independently.
  • Competency with te ao Māori, tikanga, and te reo Māori or a commitment to starting your journey and taking ownership of your learning and growth.
  • Experience in projects / initiatives which give effect to Te Tiriti principals and frameworks, and the application of Mātauranga Māori and Kaupapa Māori approaches, particularly as they apply in healthcare settings.
